Communities can maximize support for families with a Plan of Safe Care by forming Plan of Safe Care Collaboratives. A collaborative approach is a proven treatment modality for substance use, addiction, and other chronic diseases. This approach improves the health and safety outcomes of babies and families experiencing prenatal substance use by coordinating medical care, substance use treatment, and social service supports.
Through forming interdisciplinary and cross-agency collaboratives, providers are able to communicate regularly and accurately about how the family is doing and immediately connect the family to needed supports for the family.
Collaborative Structure-
- A monthly meeting of providers that serve Plan of Safe Care families.
- Verbal information sharing regarding how families are doing and what services are needed.
- Facilitated by a contracted 3rd party.
- Formed through a formal Memorandum of Understanding (MOU) and use of participant consent/release of information.
